At this time we do not accept insurance however we are in the process of credentialing with multiple insurance providers.
We provide initial patient evaluations for $99 and follow up consultations for $49. We also provide chronic disease management for diabetes, hypertension and dyslipidemia.

A current list of medical providers, medications and labwork including: complete blood count, comprehensive metabolic panel, fasting lipid panel, HgbA1C within the last year. If labwork has not been performed in the last year, we can call in labwork to be done prior to initial visit so that it can be reviewed together in person.
